Medical Resolution of Cornual Pregnancy: Preserving Fertility Through Non-invasive Method

Abstract

Introduction : Cornual pregnancy is one of the rare subtypes of ectopic pregnancy with the incidence of 2% to 4%. This is a life threating condition where women could lose life due to late diagnosis and rupture leading to excessive bleeding. While ectopic pregnancy is often managed surgically, successful medical management may offer improved outcome for future fertility.

Case: Twenty four years, primigravida admitted through OPD with diagnosis of left cornual pregnancy at 6 weeks of gestation. At the time of admission, she was hemodynamically stable. Both medical and surgical management options were given and she opted for medical management. Intraamniotic methotrexate along with multidose regimen of methotrexate and folinic acid were given. Serum beta human chorionic gonadotrophin value on day seven was found to be decreased by more than 15% percentage as compared to day four. She was discharged and followed up until three consecutive serum beta human chorionic gonadotrophin were negative.

Conclusion: Medical management of cornual ectopic pregnancy helps to avoid surgeries like cornual resection and corunostosmy and helps to preserve future fertility.
